Job summary

Option Care Health, Inc. is seeking a Clinical Transition Specialist to educate patients, families and facility staff about how services are facilitated at an alternative site and to grow referrals.

You will assess patient suitability, coordinate with intake, discharge planners and Account Managers to execute area growth plans, and ensure timely, quality transitions while maintaining patient confidentiality and compliance.

Qualifications

• Licensed RN or Pharmacist in the state of practice with healthcare experience.
• Experience building relationships with stakeholders and vendors.
• Strong customer service ethos with ability to educate and inform diverse audiences.
• Proficient in MS Office suite: Word, Excel, PowerPoint.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Interacts with area service providers to sell services and assess patient candidates for alternate site care.
• Educates patients, families, and facility staff about services and products offered by Option Care.
• Provides hands-on education to patients and caregivers to enable timely discharge and independence.
• Coordinates with internal partners to ensure smooth discharge and treatment planning.
