Job summary

Option Care Health Inc. seeks a Clinical Transition Specialist to drive patient transitions from hospital to home infusion settings.

You will use sales skills to educate patients, families, and discharge staff about services and products and proactively build relationships with referral sources. You will collaborate with intake, case management, and discharge planners to ensure accurate preadmission information and a seamless treatment plan.

Qualifications

• Must be a licensed RN or pharmacist in the state of practice with at least 2 years in healthcare.
• Experience building relationships across organizational levels.
• Knowledge of ACHC/URAC standards and healthcare regulations.

Responsibilities

• Develops and grows referrals to increase sales and patient starts.
• Educates patients and caregivers about home infusion services at discharge.
• Collaborates with intake, case management, and discharge planners.
• Manages patient transitions to home or alternative sites with timely discharge.
• Maintains confidentiality and adheres to privacy guidelines.
• Travel to partnerships facilities and hospitals to educate staff.
