Job summary

Enliven Home Infusion Specialists Arizona is seeking an experienced Registered Nurse to service home infusion and infusion center patients in Scottsdale, AZ and surrounding locations. The RNEducates patients and families on prevention and healthy habits, remaining compassionate and calm in emergent situations, while following health and safety guidelines to promote patient well-being.

Qualifications

• Background check required.
• Malpractice insurance required.
• Drug screen required.
• Associate or Bachelor Degree in Nursing (minimum).
• Minimum five years of nursing experience preferred in an Acute Care setting.
• Strong Med/Surg, ICU, ER, acute care experience.
• Home Health/Infusion experience preferred.
• Current CPR/BLS required.
• ACLS; PALS preferred.
• Skilled in accessing Ports; Central Lines; PICC lines.
• Strong IV placement skills.
• Strong organizational and communication skills.
• COVID-19 vaccination preferred by some partners/patients.

Responsibilities

• Complete daily electronic forms for each assigned patient in the EMR system.
• Provide admission, case management, and follow-up nursing visits for home infusion and infusion center patients.
• Administer ongoing care and infusions for each patient and follow physician and Clinical Manager directions.
• Develop initial treatment plans with physicians based on orders and patient assessment.
• Provide hands-on care, manage and evaluate the care plan and educate the patient under physician supervision.
• Report patient care, conditions, and progress to physician, pharmacy, and Clinical Manager.
• Implement care plan with patient and family to meet needs.
• Discharge patients after consultation with physician and Clinical Manager and finalize clinical documentation.
• Prepare medical documentation including case conferences, patient contacts, medication changes, progress updates.
• Prepare visit/shift reports, update patient records, and coordinate with other healthcare disciplines.
