Job summary

Enliven Home Infusion Specialists Arizona is seeking an experienced Registered Nurse to service home infusion and infusion center patients in Flagstaff, AZ and surrounding locations. The RN will educate patients and families on prevention and healthy habits, provide high-quality nursing care with minimal supervision, and respond to emergent situations with compassion and clinical judgment.

Qualifications

• RN with a minimum of five years nursing experience in an Acute Care setting is preferred.
• Strong Med/Surg, ICU, ER, acute care experience is required.
• Home Health/Infusion experience is preferred but not required.

Responsibilities

• Complete daily electronic forms for each assigned patient within a secured EMR system.
• Provide admission, case management, and follow-up nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions and follow-up as directed by Physician and Clinical Manager.
• Confer with physicians and pharmacies to develop initial treatment plans.
• Provide hands-on care, manage and evaluate the care plan with physician-based guidance.
• Report patient progress to physician, pharmacy, and Clinical Manager.
• Implement patient care plan with patient and family to meet needs.
• Discharge patients after consultation, completing necessary documentation.
• Prepare medical documentation including case conferences and progress updates.
• Prepare visit/shift reports and update patient records.
