Job summary

Allina Health Home CareInfusion RN Case Manager delivers infusion therapy in patients' homes and manages your own patient panel in collaboration with an interdisciplinary team to provide personalized, high‑quality care outside the hospital. You will help extend hospital-grade infusion services into the community we serve.

This casual, variable-shift role includes day/evening/night options, occasional weekends, and travel to patient homes with mileage reimbursement.

Qualifications

• Associate's or Vocational degree in Nursing
• RN with at least 2 years of experience
• Bachelor's degree in health care related field (RN, PT, OT, etc) preferred

Responsibilities

• Vascular access and medication administration per guidelines.
• Educate patients and caregivers on home infusion administration.
• Assess home environments for safe medication administration and documentation.
• Coordinate care and collaborate with interdisciplinary team to improve patient outcomes.
• Monitor for safety and escalate when needed.

Education
