Job summary

Option Care Health seeks an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home and alternate infusion suites. You will work under supervision, ensuring patient safety and adherence to physicians’ plans of care.

You will initiate, monitor, and adjust treatments, educate patients and families, document care in real time, and collaborate with the Clinical team to optimize outcomes. This role emphasizes compliance, communication, and mentorship of junior nurses.

Qualifications

• Licensed registered nurse with current state license.
• Experience in infusion/home care preferred.
• Strong documentation and communication skills.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Organizes and participates in direct patient care, performs treatments and educates patients and families/caregivers.
• Modifies plan of treatment in response to changing patient status or physician orders.
• Provides training and mentorship to Infusion Nurse I's to ensure patient safety.
• Communicates with clinical team, patients, and families to facilitate continuity of care.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
• Ensures compliance with policies, regulations, and HIPAA guidelines.
• Uses computers to validate plan of treatment orders and document care.
