Job summary

Allina Health Home Care is seeking an Infusion RN Case Manager to deliver infusion therapy in patients’ homes, manage your own panel, and collaborate with an interdisciplinary team. The role emphasizes clinical expertise, care coordination, and patient advocacy, extending high-quality infusion care beyond hospital walls.

You will work a casual, variable schedule with travel required to patient homes in Minnesota/Wisconsin areas and possible on-call shifts.

Qualifications

• RN with an associate's or vocational nursing degree and 2+ years experience.
• Must meet one of the listed RN licensure/certification options.
• CRNI and VA-BC credentials are preferred.

Responsibilities

• Manage vascular access and administer IV or subcutaneous meds.
• Educate patients and caregivers for home infusion therapies.
• Coordinate care across the home environment and team.
• Document care plans and outcomes per guidelines.
• Participate in quality improvement and practice evaluation.
• Engage in ongoing professional development and training.
