Qualifications

• Pharmacist degree (PharmD or BS) from an accredited college.
• Current pharmacist license in the state of employment.
• Minimum two years in acute-care or one year in home infusion.
• Experience with IV admixture and IV therapy, if applicable.
• Ability to collaborate with physicians, nurses, and other healthcare professionals.
• Strong communication, problem-solving, and organizational skills.

Responsibilities

• Coordinate and facilitate new referrals/admissions with nursing, customer service, and distribution.
• Review and verify new patient orders for accuracy.
• Assist in planning and implementing safe drug therapy in the home setting.
• Monitor and interpret laboratory and clinical data related to drug therapy.
• Communicate drug monitoring results to physicians and care teams.
• Maintain documentation in patient records and prepare progress reports.
