InfuCare Rx, Inc. in Aston, PA, is seeking an experienced Pharmacist to join our home infusion team. You will verify prescriptions, collaborate with physicians, nurses, and other healthcare professionals while managing all aspects of the pharmacy process to ensure safe, effective patient care.

Qualifications

• PharmD or PharmD-equivalent degree from ACPE-accredited college.
• Active pharmacist license in the state of employment.
• 2 years acute-care pharmacy or 1 year home infusion experience.
• Knowledge of IV admixture, sterile compounding and infusion therapy.
• Patient-first mindset and strong communication.

Responsibilities

• Ensure pharmacy activities comply with regulations and standards.
• Coordinate patient referrals with nursing, service and distribution teams.
• Review and verify new patient orders for accuracy.
• Consult with physicians, nurses and patients on therapy safety.
• Monitor patient outcomes and report findings to teams.
• Prepare and dispense sterile and non-sterile meds per USP standards.
• Maintain patient records and QA logs.
• Participate in QA and staff development activities.
• Maintain clinical skills through continuing education.
• Rotational on-call coverage as needed.
• Perform additional duties to support operations.
