Job summary

InfuCare Rx in Boynton Beach, FL is seeking a licensed Pharmacist to support home infusion therapies, collaborate with physicians, nurses, and staff, and ensure safe, effective drug delivery in the home setting.

You will perform distributive and clinical pharmacy activities, review orders, advise on therapy, and participate in sterile compounding and documentation while maintaining compliance with regulations.

Qualifications

• PharmD or B.S. in Pharmacy from an accredited college.
• Active pharmacist license in the state of employment.
• Minimum 2 years acute-care or 1 year home infusion experience.
• Experience with intravenous admixture and intravenous therapy.

Responsibilities

• Coordinates referrals with nursing, CS, and distribution.
• Reviews and verifies all new patient orders and documents.
• Assists physicians and nurses in planning patient care at home.
• Monitors drug therapy outcomes and interprets data.
• Programs infusion control devices and sterile compounding processes.
• Maintains patient records and prepares progress reports.
• Participates in on-call rotation when applicable.
• Provides drug information to patients and clinicians.
