Job summary

Holly Hill Pharmacy, an independent pharmacy in Holly Hill, Florida, seeks a full‑time Pharmacist to join our Home Infusion team. Typical hours are 9:00 a.m.–5:30 p.m. Monday–Friday, with an every‑other‑week on‑call rotation.

You will prepare and oversee home‑infusion and specialty‑infusion medications, participate in quality‑assurance activities, counsel patients, coordinate insurance coverage, and collaborate with prescribers and referral sources.

Qualifications

• BS in Pharmacy or PharmD from an accredited school of pharmacy.
• Current, unrestricted pharmacist license in Florida.
• Working knowledge of USP <797> requirements.
• Strong clinical judgment and problem‑solving skills.
• Excellent communication, organizational, and time‑management skills; ability to work independently and as part of a team.
• Ability to manage multiple priorities and adapt to changing patient needs.
• Computer proficiency and ability to learn pharmacy management and other healthcare software.
• Commitment to providing excellent patient care and customer service.
• Must not be excluded or debarred from federal healthcare programs.
• No pending charges or convictions involving controlled substances.

Responsibilities

• Prepare, verify, and dispense home‑infusion and specialty‑infusion medications.
• Oversee technicians involved in preparation and compounding.
• Maintain USP <797> compliance and sterile compounding standards.
• Review prescriptions for clinical appropriateness and interactions.
• Counsel patients and caregivers on administration and expectations.
• Communicate with physicians, nurses, and other healthcare professionals.
• Monitor patient therapy and laboratory results when applicable.
• Assist with third‑party claims and prior authorizations.
• Participate in quality assurance and performance improvement programs.
• Manage inventory, purchasing, storage, and expiration monitoring.
• Maintain complete patient and pharmacy documentation.
• Comply with federal and state laws and regulations.
