Job summary

Amerita, Inc. is seeking a Pharmacist to join our Pharmacy team in Apex, NC. The Pharmacist will report to the Pharmacy Manager and work in our City, State branch, supporting home infusion patients and clinical drug therapy management.

Required: B.S. in Pharmacy, active Pharmacist license, 2+ years in acute care or 1+ year in home infusion; experience with IV drug compounding preferred. Flexible schedule with on-call evenings and weekends and travel up to 25% may be required.

Qualifications

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
• Registered Pharmacist License in state(s) of practice.
• Minimum of two (2) years of recent experience in an acute-care setting or one (1) year in home infusion therapy.
• Experience with compounding infused drugs.
• Proven record of evaluating and monitoring drug therapies for home care patients.
• Flexible schedule with the ability to work on-call evenings and weekends on an as-scheduled basis.
• Travel: 0-25% - Minimal travel may be required.

Responsibilities

• Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
• Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
• Adheres to accreditation standards and participates in quality assurance activities.
• Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution.
• Reviews, receives and verifies all new patient orders and documents on the respective order form.
• Assists the physician and other involved healthcare professionals with planning and implementing patient care in the home; setting therapeutic objectives; establishing monitoring parameters; and preparing regular progress reports.
• Conducts and documents initial and ongoing assessment of patient, drug and disease information to identify drug-related problems.
• Monitors outcomes of drug therapy by evaluating laboratory and clinical data.
• Provides consultation and answer inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
• Maintains documentation in patient records of all clinical activities and communications.
• Processes and checks medications, supplies and equipment prior to dispensing.
• Maintains clinical skills by reviewing literature and attending in-services, seminars and conferences.
• Compounds sterile and non-sterile preparations following aseptic technique where state laws permit.
• Ensures clean room areas and equipment comply with USP 797 and USP 800 standards.
• Oversees Pharmacy technician functions as required by Pharmacy Board rules.
