Qualifikationen

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
• Registered Pharmacist License in state(s) of practice.
• Minimum of two years of acute-care experience or one year in home infusion.
• Experience with compounding infused drugs.
• Proven record of evaluating and monitoring drug therapies for home care patients.
• Excellent verbal and written communication skills.
• Flexible schedule with evening/weekend on-call availability.
• Travel 0-25% may be required.

Aufgaben

• Evaluate and monitor drug therapy for home care patients with physicians and nurses.
• Adhere to all policies, procedures, and laws governing pharmacy practice.
• Coordinate referrals and admissions with care teams.
• Review and verify patient orders and document accordingly.
• Assist providers with planning patient care at home and monitoring therapy.
• Document and monitor patient data to identify drug-related problems.
• Provide drug information and answer inquiries from patients and clinicians.
