Job summary
Amerita, an affiliate of BrightSpring Health Services, is hiring a Pharmacist to join our home infusion pharmacy team. You will evaluate and manage drug therapies for home care patients, collaborate with physicians, nurses, and distribution, and ensure compliance with USP standards.
The role requires a Doctor of Pharmacy or BS in Pharmacy with a valid license, and experience in acute care or home infusion. Flexibility for varied schedules and on-call coverage is expected, with competitive
Qualifications
- Pharmacy degree required; PharmD preferred.
- Licensed Pharmacist in applicable state(s).
- 2 years acute-care experience or 1 year home infusion experience.
- Experience with compounding infused drugs.
- Excellent verbal and written communication skills.
Responsibilities
- Evaluates drug therapy for home care patients with physicians and nurses.
- Adheres to policies, procedures and laws governing pharmacy practice.
- Coordinates referrals and admissions with nurses, customer service and distribution.
- Reviews, verifies and documents new patient orders.
- Assists physicians with home care planning and monitoring of drug therapy.
- Monitors outcomes by reviewing laboratory and clinical data.
- Maintains thorough documentation in patient records.
