Amerita, Inc. is seeking a Pharmacist to join our pharmacy team in the Pawtucket area.

You will perform pharmaceutical care activities for home care patients, collaborate with physicians and nurses, and participate in continuous professional development and career progression discussions. The role requires a Doctor of Pharmacy or PharmD (BKBS/BS Pharm acceptable in some cases), active pharmacist licensure, and experience with compounding infused drugs.

Qualifications

• Pharmacist must be licensed in applicable states.
• Minimum two years in acute-care settings or one year in home infusion.
• Experience compounding infused drugs preferred.
• Strong documentation and communication skills required.
• Ability to work with healthcare teams and coordinate care.

Responsabilités

• Evaluate and monitor drug therapy for home care patients.
• Adhere to policies, procedures, and state/federal regulations.
• Coordinate referrals and admissions with nurses and distribution.
• Review and verify patient orders and document accordingly.
• Assist physicians and clinicians with home care planning and monitoring.
• Document progress and outcomes of drug therapy for patients.
• Provide drug information consultations to patients and healthcare professionals.
• Maintain thorough patient records and logs.
• Oversee pharmacy technician activities as needed.
