Job summary

Amerita, Inc. is seeking a Pharmacist to join our home infusion pharmacy team. The Pharmacist will report to the Pharmacy Manager and work in our City, State branch, supporting patient care and ensuring compliance with USP standards.

Responsibilities include evaluating drug therapy, coordinating referrals, and documenting all clinical activities. A pharmacist license and experience in infusion or acute care are preferred; a flexible schedule is required.

Qualifications

• Pharmacist must hold a valid state license.
• Experience in home infusion or acute care is preferred.
• Excellent communication with patients, families, and healthcare professionals.

Responsibilities

• Evaluate and monitor drug therapy for home care patients with clinicians.
• Adhere to policies, procedures, and regulatory standards.
• Coordinate referrals and admissions with nurses, customer service, and distribution.
• Review and verify all new patient orders and document accordingly.
• Assist physicians and other professionals with patient care planning and monitoring.
