Job summary

Amerita is seeking a Pharmacist to join our Pharmacy team in our North Carolina branch. You will perform pharmaceutical care activities for home infusion patients and collaborate with physicians, nurses, and distribution staff to ensure optimal drug therapy outcomes.

The role requires a Doctor of Pharmacy (or BS in Pharmacy) with a valid pharmacist license in the practice states, and 2+ years in acute care or 1 year in home infusion.

Qualifications

• Doctor of Pharmacy degree preferred; Bachelor of Science in Pharmacy required.
• Registered Pharmacist license in state(s) of practice.
• Minimum of two years in acute-care or one year in home infusion.
• Experience with compounding infused drugs.
• Proven ability to evaluate and monitor drug therapies for home care patients.
• Excellent verbal and written communication skills.
• Flexible schedule with on-call evenings and weekends.

Responsibilities

• Evaluates and monitors drug therapy for home care patients with clinicians.
• Adheres to all pharmacy policies, procedures and regulations.
• Coordinates referrals and admissions with nurses and distribution.
• Reviews and verifies new patient orders and documents accordingly.
• Assists physicians and other professionals with home care planning and therapeutic objectives.
• Conducts assessments to identify drug-related problems and needs.
• Monitors outcomes by reviewing lab and clinical data.
• Provides drug information to patients, families, and healthcare professionals.
• Maintains thorough documentation in patient records.
• Prepares and checks medications and equipment prior to dispensing.
• Maintains and updates knowledge through literature reviews and trainings.
• Oversees pharmacy technician functions as required.
