Job summary

Amerita is seeking a Pharmacist to join our home infusion pharmacy team. You will evaluate and monitor drug therapies, collaborate with physicians and nurses, and ensure compliance with all laws and USP standards.

The role involves coordinating referrals, reviewing orders, and supporting patient care planning in the home setting. Monday–Friday 9:00am–5:30pm; the position requires strong communication skills, attention to detail, and the ability to work across multidisciplinary teams.

Qualifications

• Bachelor of Science in Pharmacy (Doctor of Pharmacy preferred).
• Licensed Pharmacist in state(s) of practice.
• Minimum 2 years in acute-care or 1 year in home infusion therapy.
• Experience with compounding infused drugs.
• Proven record of evaluating and monitoring drug therapies for home care patients.
• Excellent verbal and written communication skills.
• Flexible schedule with on-call evenings and weekends.
• Travel up to 25% may be required.

Responsibilities

• Evaluates and monitors drug therapy for home care patients with physicians, nurses and other healthcare professionals.
• Adheres to organization policies, procedures, and all applicable laws and regulations.
• Coordinates and facilitates new referrals and admissions with nurses, customer service and distribution.
• Reviews and verifies new patient orders and documents on the order form.
• Assists physicians and other professionals with care planning and monitoring in the home.
• Documents and maintains patient drug and disease information to identify drug-related problems.
• Compounds sterile and non-sterile preparations, following USP 797 and 800 where permitted.
• Oversees Pharmacy technicians as required by Pharmacy Board rules.
