Job summary

Amerita, an affiliate of BrightSpring Health Services, seeks a Pharmacist to join our home infusion pharmacy team in our San Antonio, TX area branch. You will evaluate and monitor drug therapy for home care patients, collaborating with physicians, nurses and other healthcare professionals to ensure optimal outcomes.

The role requires a BS in Pharmacy (PharmD preferred), a valid pharmacist license, and a minimum of two years acute-care or one year home infusion experience.

Qualifications

• BS in Pharmacy required; PharmD preferred.
• Licensed pharmacist in state(s) of practice.
• Minimum two years of acute-care experience or one year in home infusion.
• Experience with compounding infused drugs.
• Proven track record evaluating and monitoring drug therapies for home care patients.
• Strong organizational and time-management skills.
• Excellent verbal and written communication skills.
• Flexible schedule with on-call evenings and weekends.

Responsibilities

• Evaluates and monitors drug therapy for home care patients with physicians, nurses and other healthcare professionals.
• Complies with state/federal laws and accreditation standards governing pharmacy practice.
• Coordinates and facilitates referrals and admissions with nurses, customer service and distribution.
• Reviews and verifies new patient orders and documents on orders.
• Assists physicians and healthcare professionals with home care planning and therapy objectives.
• Monitors outcomes of drug therapy using laboratory and clinical data.
• Provides drug information and answers inquiries from patients and families.
