Zusammenfassung

Soleo Health seeks a Full-time Infusion Nurse to support infusion patients in Desert Ridge, AZ and surrounding area. You will provide primary patient care, assessments, care planning, and education in the home or alternate site setting, coordinating with pharmacists, prescribers, dietitians, and care partners.

Responsibilities include evaluating home infusion safety, developing care plans, scheduling per policy, and maintaining documentation.

Qualifikationen

• Current RN license by the state Board of Nursing, in good standing.
• RN degree, BSN preferred from an accredited institution.
• Advanced certification in specialty areas (CRNI/ONS/IgRN) preferred but not required.

Aufgaben

• Provide direct infusion patient care and coordinate with care team.
• Educate patient/care partner for safe home infusion administration.
• Assess home environment and ensure safety for prescribed care.
• Develop and evaluate plan of care with the team.
• Manage scheduling and communicate changes per policy.
• Coordinate patient care via approved company platform.
• Participate in the nurse on-call rotation for 24/7 support.
• Document all patient encounters and care activities per policy.
