Job summary

Option Care Health LLC is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or infusion suite. The RN will ensure patient safety under a physician’s plan of care and work under supervision.

The role requires active RN license, at least 1 year of nursing experience, and current CPR certification. Travel up to 10% may be required; benefits include medical, dental, vision, PTO, and 401(k).

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments and educates patients and families.
• Assesses patient needs and status during visits and revises plans as needed.
• Provides training and mentorship to Infusion Nurse I’s to ensure safety and compliance.
• Communicates with clinical team and physicians to ensure continuity of care and timely documentation.
• Coordinates discharge planning and prepares discharge summaries with clear patient instructions.
