Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or infusion suite, under physician plan of care. You will ensure patient safety and documentation, working under supervision.

The role requires an active RN license, CPR, and 1 year of infusion nursing experience. Travel up to 100% is required to support patients across assigned regions; strong communication with clinical teams is essential.

Qualifikationen

• Active RN license in state of practice.
• Minimum of 1 year of infusion nursing experience.
• Current CPR certification required.

Aufgaben

• Initiates and implements intermediate nursing plan of care treatments and educates patients and families.
• Assesses patient needs and status at each skilled visit and documents changes.
• Provides training and mentorship to Infusion Nurse I’s as needed.
• Communicates with clinical team and physicians to coordinate care and billing.
• Prepares discharge instructions and summaries for transfer of care.
• Ensures compliance with agency policies, HIPAA, and regulatory standards.
