Job summary

Soleo Health is seeking a full-time Infusion Nurse to support infusion patients in the Dacula, GA area. The role involves direct patient care, assessments, care planning, and education in home or alternate sites, with on-call rotation and driving between patient homes.

Qualifications

• Current RN license by the applicable state's Board of Nursing, in good standing.
• BSN degree preferred from an accredited institution.
• Infusion therapy experience is preferred; ICU/Cardiac care experience is an asset.
• Participation in an on-call rotation with nights and weekends possible.
• Proficient with Microsoft Outlook and Office Suite.

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communications with care team.
• Evaluate and educate patient and care partner to safely administer home infusion therapy.
• Assess home environment and plan care with the team.
• Develop and evaluate the care plan, document encounters, and ensure regulatory compliance.
• Coordinate patient care using approved company platform and participate in on-call rotation.
