Soleo Health Inc. seeks a Full-time Infusion Nurse to support infusion patients in Conyers, GA and surrounding area. You will provide direct care, educate patients, and coordinate home infusion services.

Qualifications

• Active RN license in state Board of Nursing in good standing.
• BSN degree preferred; advanced certification optional.
• Minimum 3 years nursing experience, infusion therapy preferred.
• Experience in ICU or Cardiac Care preferred.
• Proficient with Outlook and MS Office.

Responsibilities

• Provide direct care for all assigned patients including initiation and maintenance of communications with care team members.
• Educate patient/caregiver to safely administer home infusion therapy and provide instructions as needed.
• Evaluate home environment and identify interventions for safe prescribed care.
• Develop and evaluate plan of care with other team members.
• Manage schedule via company platform and communicate changes to team.
• Coordinate patient care via approved company platform.
• Participate in the nurse on-call rotation to ensure continuous support.
• Document all patient encounters and care activities per policy.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
