S Soleo Health is seeking a Full-time Infusion Nurse to support infusion patients in Conyers, GA and surrounding areas. The role provides direct patient care, education, and coordination with the care team in home or alternate sites.

The ideal candidate will handle assessments, care planning, and documentation while participating in on-call coverage and maintaining knowledge of Soleo therapies and standards.

Qualifications

• Assessments, care planning, evaluation and education for patients receiving infusion therapy.
• Provide direct patient care including initiation and maintenance of communication with care team.

Responsibilities

• Provide direct care for all assigned patients including initiation and maintenance of communications with care team members.
• Evaluate and educate patient/care partner to safely administer home infusion therapy.
• Evaluate home environment and identify interventions to ensure safe home care.
• Develop and evaluate the plan of care with team members.
• Manage schedule and communicate changes per policy.
• Coordinate patient care via approved company platform.
• Participate in nurse on-call rotation to provide continuous support.
• Document all patient encounters and care activities per policy.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment and other required documents.
• Work with departments to complete patient documents in a timely manner.
