Job summary

Option Care Health, Inc. is seeking a Clinical Transition Specialist to educate patients and families and to coordinate the transfer of care from hospital to home or alternate sites.

You will build relationships with discharge planners, case managers and referral sources to grow patient starts and meet sales goals. The role focuses on using sales techniques to facilitate understanding of services, ensure timely discharge, and collaborate with Account Managers to drive growth and quality patient

Qualifications

• Licensed RN or licensed Pharmacist with clinical healthcare experience.
• Minimum of 2 years in the healthcare industry.
• Excellent communication and relationship-building skills.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Interacts with area service providers to sell Option Care services that aid discharging patients.
• Educates patients, families and facility staff about Option Care services and products.
• Provides hands-on, in-person education to patients and caregivers to support timely discharge and independence.
• Coordinates discharge process with medical team and family to implement an effective treatment plan.
• Collects, reviews and secures pre-admission information and related signoffs.
• Partners with Account Manager to create and execute area business and growth plans.
• Delivers education to patient sources with marketing staff to promote services.
• Reviews medical records to obtain history and insurance payor information for patient registration.
• Facilitates transfer of orders via phone, fax, and e-prescribing for seamless transitions.
