Job summary

Option Care Health is seeking a Clinical Transition Specialist to educate patients and care teams about home infusion services and to place patients in appropriate alternative-site care settings. The role involves building relationships with hospitals, discharge planners, and referral sources to meet sales goals.

Qualified candidates are RN or Pharmacist licensed, with 2+ years in healthcare, strong communication, and the ability to travel to care facilities.

Qualifications

Experience establishing relationships at all levels of an organization and with vendors.
Experience applying knowledge of standards (ACHC, URAC) for services offered.
Experience delivering excellent customer service to internal and external customers.
Proficient computer skills: internet, email, Word, Excel, PowerPoint.
Ability to plan, organize and present.
Must have reliable transportation for travel to facilities and hospitals.

Responsibilities

Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
Interacts with area service providers to sell Option Care services to discharging patients.
Educates patients, families and facility staff about services and products provided by Option Care.
Provides hands-on, in-person education to patients and caregivers to support timely discharge and independence.
Coordinates discharge planning and ensures implementation of the treatment plan with internal partners.
