Job summary

Alivia Infusion Services is seeking a Clinical Services Coordinator to provide administrative and operational support for referrals, admissions, coordination, and follow-up of patients receiving home infusion services.

You will serve as the central point of contact among clinical staff, patients and families, physicians and provider offices, health plans and networks, the pharmacy, and other areas related to admission and service coordination.

Qualifications

• Associate degree or college coursework in related field preferred.
• 1–3 years in home health care, home infusion, or similar settings.
• Experience with referrals, data entry and patient records.
• Familiarity with health plan authorizations and service coordination.
• Proficient in MS Office applications.
• Knowledge of HIPAA and PHI handling.

Responsibilities

• Receive and review medical orders, referrals, authorizations and documentation.
• Identify the source of referrals and forward to appropriate supervisor or region.
• Verify physician and provider information for completeness and accuracy.
• Review authorization documentation and verify dates and scope.
• Identify discrepancies and escalate promptly to staff.
• Coordinate admission timelines and document delays with justification.
• Prepare and maintain administrative documentation for admissions.
• Assist with scheduling and administrative coordination of services.
• Confirm scheduled visits and services with patients/families.
• Answer calls from patients, families, and providers professionally.
• Follow up on administrative requests and refer clinical matters appropriately.
• Keep records up-to-date and compliant with AIS policies.
• Prepare reports on referrals, admissions, authorizations, and scheduling.
