Job summary

FFF Enterprises is seeking a Clinical Nurse Coordinator to support safe and timely admission to NuFactor services. You will coordinate nursing schedules for an assigned caseload and serve as a clinical resource for subcontracted agencies and internal staff.

Collaboration with physicians and families is essential to obtain documentation for care coordination, reimbursement, and continuity of care, while promoting safe transitions to home care services and ongoing infusion nursing support.

Qualifications

• High school diploma or equivalent.
• Completion of an accredited practical nursing or registered nursing program.
• Minimum of three nursing experience.

Responsibilities

• Serve as a central clinical resource to internal and external staff regarding nursing and clinical matters, including after-hours telephone-based support as required.
• Collaborate with physicians, clinical staff, patients and families to obtain medical documentation for treatment coordination, reimbursement justification, and continuity of care.
• Facilitate safe and effective transition of patients to home care services by evaluating placement needs and home environment suitability.
• Assist with evaluation, coordination, and ongoing support of home infusion nursing services.
• Support development, coordination and management of subcontracted nursing services and independent contract nursing resources.
• Assist in development, review and revision of department policies and procedures as needed.
• Review nursing documentation for therapy appropriateness, outcomes, completeness and adherence to standards.
• Demonstrate proactive approach to responsibilities, managing multiple priorities and deadlines.
• Participate in on-call schedules, special projects, and department initiatives.
• Maintain knowledge of industry trends and continuing education through literature review and professional development.
