Job summary

Community Hospice is seeking a Registered Nurse Case Manager to plan and manage the nursing care for an assigned caseload in home or long-term care settings. You will collaborate with the interdisciplinary team to achieve patient- and family-directed goals using a holistic, patient-centered approach.

Responsibilities include education, care coordination across sites, and ensuring timely responses to changing patient needs, with flexibility to work across care settings.

Qualifications

• Excellent communication skills (verbal, written, non-verbal)
• Understanding of hospice and palliative nursing principles
• Cultural competence and ethical decision-making in care delivery
• Ability to set professional boundaries and resolve team conflicts
• Strong critical thinking and clinical judgment
• Proficient in time management, organization, and accountability
• Knowledge of infection control and standard precautions

Responsibilities

• Assess and identify patient and family goals and needs.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Participate in team meetings and ethical discussions.
• Educate patients and caregivers about disease progression, medications, nutrition, and hospice philosophy.
• Coordinate timely and effective responses to changing or urgent patient needs.
• Maintain accurate, timely, and compliant documentation.
• Support and educate team members, patients, families, and the broader community.
• Participate in internal quality improvement initiatives.
• Facilitate interdisciplinary group (IDG) meetings in the absence of the manager.
• Perform other duties as assigned.
