Nurse on Call of South Florida, LLC seeks a dedicated care coordination professional to support patients transitioning from hospital to home health in Florida. You will facilitate effective communication among the patient’s care team and the Home Health agency while addressing patient and caregiver needs.

Responsibilities include meeting with patients, coordinating referrals, and ensuring CMS criteria are met.

Qualificações

• Minimum two years of experience in home health.
• Experience in transitional care or case management preferred.
• Clinical license required, LPN license preferred.
• Proficient in Microsoft Office including Outlook.

Responsabilidades

• Provide individualized service during the transition from hospital to home health.
• Maintain positive relationships with hospital case management and physicians.
• Meet with patients to discuss home health benefits and develop a preliminary plan of care.
• Coordinate referrals from hospital through home health agency and CMS criteria.
• Ensure required documentation travels with referrals for billable episodes of care.
• Serve as a resource to hospitals regarding home health services.
• Deliver home health education to hospital teams as requested.
• Problem-solve and escalate to leadership as needed.
• Develop rapport with discharge planners and key contacts; visit them regularly.
• Lead new initiatives and ensure successful program implementation.
• Perform other duties as assigned.
