Nurse on Call, LLC in Fort Myers, FL is seeking a Transitional Care Coordinator to ensure safe hospital-to-home health transitions. You will coordinate communication among patients, families, hospital teams, and home health agencies to address patient and caregiver needs and verify CMS criteria for services.

The role requires at least two years in home health, a clinical license (LPN preferred), and proficiency in Microsoft Office, including Outlook.

Formación

• Minimum two years of experience in home health.
• Experience working in transitional care or case management preferred.
• Clinical license required, LPN license preferred.
• Proficient in Microsoft Office including Outlook.

Responsabilidades

• Provides individualized service delivery throughout the transition process.
• Creates and maintains a positive relationship with the hospital Case Management team.
• Meets with patients to discuss the home health benefit and develop a preliminary plan of care.
• Manages the referral process from the hospital through home health agency acceptance and CMS criteria.
• Ensures documentation is sent with the referral to support a billable episode of care.
• Serves as a resource to the hospital regarding home health services.
• Develops home health educational in-services to hospital teams as requested.
• Develops rapport with physicians and discharge planners and visits them regularly.
• Leads new initiatives within the assigned area and ensures successful implementation.
• Performs other duties as assigned.
