Job summary

AccentCare is seeking a Transition Care Navigator to promote our homecare services and coordinate patient transitions. You will engage with physicians, hospitals and facilities, acting as the liaison to ensure smooth communication and strong client relationships.

The role requires an active RN or LPN license, solid knowledge of Medicare/home health regulations, and experience in the local market; prior medical sales is a plus. We offer competitive benefits and a collaborative team environment.

Qualifications

• Current license/registration by state of hire as an RN or LPN.
• Knowledge of governmental home health regulations and Medicare requirements.
• Experience in local market; care plan development knowledge.
• Previous medical sales experience preferred.

Responsibilities

• Call on physicians, hospitals, nursing facilities and providers to promote AccentCare homecare services.
• Be an expert regarding AccentCare services and ensure effective communication with providers.
• Act as liaison to clients to resolve problems and provide information on services.
• Partner with Account Executives, ED, and RSM to develop and implement local market business plans.
• Maintain organized client account information, track sales data, identify key accounts and issues.
