Nurse on Call Home Care, LLC in Florida is seeking a Registered Nurse to provide coordinated skilled nursing in patients' homes on an intermittent basis, collaborating with the interdisciplinary team to meet patient goals and maintain high-quality care.

The role follows physician orders and works under the Home Health Director, offering leadership to aides and LPNs/LVNs, while participating in care conferences and performance improvement activities.

Qualifications

• Graduate of an accredited school of nursing.
• Current license as a Registered Nurse in good standing for the state.
• BSN preferred.
• One year experience as a registered nurse in a clinical setting required.
• Acute care and home care experience recommended.

Responsabilités

• Assess home health patients' physical, psychosocial, and environmental needs.
• Complete OASIS, assessment, and visit paperwork per policy.
• Communicate significant findings to Home Health Director and physician.
• Identify and report problems or adverse reactions in patient care.
• Develop and implement the Plan of Care with the physician, patient, family and team.
• Supervise and provide direction to home health aides and LPNs/LVNs.
• Participate in interdisciplinary patient care conferences and agency improvement processes.
• Receive referrals and coordinate timely communication.
