Job summary

Inspired Homecare is seeking a compassionate Clinical Liaison to facilitate smooth transitions of care from hospitals and rehab centers to home health care. You will serve as the primary bridge among hospitals, rehabilitation centers, physicians, and our intake team.

Responsibilities include coordinating transitions, managing referral relationships, educating patients and families, and driving market growth by leveraging clinical networks.

Qualifications

• Active unencumbered professional license as RN, LPN, PT or OT; social workers also considered.
• Minimum of two years of clinical experience in acute care, rehab, or home health.
• Ability to communicate complex clinical details clearly to non-medical family members.

Responsibilities

• Transition Coordination: collaborate with hospital case managers, discharge planners, social workers, and physicians to ensure safe, efficient transitions to home health care.
• Referral Relationship Management: build and maintain trusted relationships with key healthcare decision-makers.
• Patient and Family Education: meet with patients and families before discharge to explain home health care processes and address concerns.
• Market Growth: meet or exceed monthly referral and admission goals by identifying new opportunities within clinical network.
