Job summary

Residential Home Health and Hospice in Shiawassee County is seeking a licensed Speech-Language Pathologist to provide skilled services in patients’ homes within an assigned territory. You’ll evaluate, treat, and educate patients and families, collaborating with a supportive interdisciplinary team for safe, quality care.

Qualifications

• Active Speech Language Pathologist license in the state you work.
• Minimum 1 year of experience as an SLP delivering patient care.
• Valid driver’s license with ability to travel ~20% of workday.

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life.
• Educate and empower patients and caregivers on therapeutic exercises.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

SLP Qualifications

• Active Speech Language Pathologist, SLP license in the state you work
• One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
