Job summary

Graham Healthcare Group's Residential Home Health and Hospice is hiring a Speech Language Pathologist (SLP) to provide skilled care in patients' homes across Plainfield, Aurora, Naperville and surrounding areas. This role offers a $15,000 total bonus potential, including a $10,000 first-year success bonus and a $5,000 sign-on bonus paid within 18 months, contingent on performance.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life
• Educate and empower patients and caregivers on therapeutic exercises
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

SLP Qualifications

• Active Speech Language Pathologist, SLP license in the state you work
• One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
