Job summary

Residential Home Health and Hospice is seeking a Speech Language Pathologist (SLP) for home health services in Genesee County, MI and surrounding areas. You will evaluate, treat, and support patients in their homes, collaborating with an interdisciplinary team.

Requires active SLP license in the state, at least 1 year of experience, and a valid drivers license to travel about 20% of your workday. The role offers meaningful autonomy and growth within a nationally recognized home health leader.

Qualifications

• Active Speech Language Pathologist with current SLP license in the state of work.
• Minimum of 1 year experience as an SLP delivering patient care.

Responsibilities

• Deliver skilled, patient-centered care in home health settings.
• Perform comprehensive evaluations of speech, language, cognitive-communication and swallowing.
• Educate patients and caregivers on exercises and therapy plans.
• Complete EMR documentation accurately and timely.
• Travel to patient homes within assigned territory.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life
• Educate and empower patients and caregivers on therapeutic exercises
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

SLP Qualifications

• Active Speech Language Pathologist, SLP license in the state you work
• One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
