Job summary

Enhabit Home Health & Hospice is seeking a Speech-Language Pathologist to evaluate and provide skilled therapy in home settings under physician supervision. Patients may live in single or multi-family homes, assisted living, or memory care environments.

The role involves assessing language impairments, motor speech disorders, dysphagia, and cognitive communication challenges, with care coordination alongside the attending physician to improve home-based patient outcomes.

Qualifications

• Valid state driver’s license.
• Automobile liability insurance.
• Current SLP license for the state of employment.
• Ability to drive in all weather conditions.

Responsibilities

• Provide skilled speech-language therapy in patients’ homes.
• Assess and treat language impairments, motor speech, dysphagia and cognitive communication issues.
• Collaborate with attending physicians for patient care in home settings.
• Support patients in single or multi-family homes, assisted living or memory care as needed.
