Job summary

CenterWell Home Health, a Humana company, is seeking a licensed Speech-Language Pathologist to provide care in patients' homes or facilities. You will evaluate and treat speech, language, voice, hearing and swallowing disorders, develop care plans, and document patient progress with a focus on quality outcomes.

The role requires an ASHA-certified credential, valid state license, CPR, and a valid driver’s license with reliable transportation. Experience in home health is preferred.

Qualifications

• Minimum six months experience as a speech therapist/speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech-language pathology service to patients in the home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent and minimize impact.
• Administer and interpret diagnostic tests and therapy applications including audio logic screening.
• Document changes in patient condition and response to treatment.
