Qualifications

Responsibilities

• Performs evaluations, determines SLP diagnoses, and develops a plan of care.
• Provides reassessments including OASIS assessments and updates plans with the physician.
• Provides therapeutic treatments and evaluates equipment needs; documents progress.
• Collaborates with a multi-disciplinary team; participates in meetings and case conferences.
• Modifies treatment plans to maximize outcomes and discharge readiness.
• Coordinates with Home Health Aides for supervisory visits and training.
