The clinician will perform OASIS assessments, coordinate with an interdisciplinary team, and contribute to quality outcomes through documentation and patient progress notes.

Qualifications

• Master's Degree in Speech-Language Pathology is required.
• 1 year SLP experience preferred.
• Licensure: SLP license (ASHA) and VDHP licensure upon hire.
• CPR/BLS certification within 30 days.

Responsibilities

• Performs evaluations, diagnoses, develops plan of care, and provides speech therapy to restore/maintain speech, language, motor, and swallowing abilities.
• Provides reassessments including OASIS assessments at appropriate time points.
• Updates plan of care in consultation with the physician and interdisciplinary team.
• Provides therapy and assesses equipment needs; prepares clinical and progress notes.
• Plans for discharge and communicates progress to the patient and care team.
• Coordinates with Home Health Aides and participates in interdisciplinary meetings and case conferences.
