Job summary

Pathwell Health is seeking a Speech Language Pathologist for Home Health in Virginia. You will assess and treat speech, language, swallowing, and cognitive disorders, develop treatment plans, and monitor patient progress in collaboration with the care team.

You will educate patients and families, document progress, and participate in team meetings. A valid Virginia SLP license, Master’s degree, and access to a vehicle are required; CPR is preferred.

Qualifications

• Master's degree required in Speech Therapy or Speech-language Pathology.
• Virginia state license in Speech Therapy required.
• Driver’s license, car insurance and car registration.
• CPR certification preferred.

Responsibilities

• Provide speech therapy services within plan of care and in coordination with the health care team.
• Evaluate function and assist in developing therapy plan of care.
• Treat patients to improve communication, swallowing, and cognition.
• Document progress notes after each visit and report outcomes.
• Instruct patient, family, and caregivers in therapy activities.
