Job summary

Discovery at Home Gulf Coast is seeking a licensed Occupational Therapist to provide home health and outpatient services across Florida. You will evaluate clients, create individualized treatment plans, monitor progress, and educate patients and families on mobility and safety.

A valid FL license and strong documentation skills are required. Experience in home health and knowledge of Medicare/Medicaid guidelines is preferred.

Qualifications

• Must hold a Florida license or be eligible for licensure.
• Home health experience is preferred.

Responsibilities

• Conduct initial evaluations to assess patients' needs and develop individualized treatment plans.
• Develop and implement treatment plans based on patient needs and goals.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and caregivers on techniques to improve mobility, reduce pain, and prevent injuries.
• Maintain accurate and timely documentation of patient progress.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Provide home health and/or outpatient services, including supervisory visits, to ensure optimal outcomes.
