Job summary

Residential Home Health and Hospice is seeking a Speech Language Pathologist (SLP) to provide skilled care in patients’ homes around Downers Grove, IL. The role offers a substantial bonus potential and a path for growth within a nationally recognized home health provider.

The position involves evaluating and treating communication and swallowing disorders, educating patients, collaborating with a multidisciplinary team, and documenting care in the EMR system.

Qualifications

• Active Speech Language Pathologist with a valid SLP license in the work state.
• Minimum of 1 year experience as an SLP delivering patient care.
• Due to field-based home health work, a valid driver's license and ~20% travel time.

Responsibilities

• Deliver skilled, patient-centered care in patients' homes.
• Perform comprehensive evaluations of speech, language, cognition, and swallowing.
• Develop and implement individualized treatment plans.
• Educate patients and caregivers on exercises and strategies.
• Maintain compliant EMR documentation.
• Travel to patient homes within assigned territory.
