Job summary

PruittHealth is seeking a Registered Nurse to join its Home Health Services team as a part-time RN. The role focuses on delivering skilled nursing for home-based patients under physician supervision, ensuring high-quality care in a home setting.

The RN will supervise HHAs/LPNs, coordinate care plans with the interdisciplinary team, and participate in performance improvement initiatives while maintaining compliance with regulations and patient rights.

Qualifications

• RN home health experience required.
• Liability insurance to complete home visits.
• Current, active RN licensure in state of practice.
• BSN preferred (optional).

Responsibilities

• Provide and coordinate skilled nursing care for home health patients under physician supervision.
• Supervise Home Health Aides and LPNs on the care team.
• Develop, integrate, and manage the plan of care with the interdisciplinary team and physician.
• Conduct field visits to initiate care and monitor outcomes and quality improvement.
• Document patient health status and care activities accurately and timely.
