Job summary

Acacia Health (Palliative, Home Health, & Hospice) is seeking a compassionate Registered Nurse for home health visits across Orange County. You will assess patients, implement care plans, administer treatments, and educate families in their homes. Requirements: CA RN license, CPR/BLS, CA driver license, and at least 1 year of home health experience.

Pay is $52–$55 hourly. This full-time role involves driving to patients' homes and coordinating care with the physician and family.

Qualifications

• CA RN license required.
• CPR/BLS certification required.
• CA driver's license required.
• At least 1 year of home health experience required.

Responsibilities

• Visits patients to assess and evaluate patient’s status.
• Initiates the plan of care and makes revisions as needed.
• Administers medications and treatments as prescribed by the physician.
• Provides health care instructions to the patient & family.
• Completes accurate clinical notes in electronic EHR.
• Works in cooperation with the family/caregiver.
