Job summary

Acacia Health (Palliative, Home Health, & Hospice) seeks a compassionate Registered Nurse to plan, direct, and coordinate hospice services in patients’ homes and communities. The RN will assess needs, implement plans of care, and work closely with families to achieve comfort and dignity.

With strong clinical skills and CPR certification, you will manage care plans, document in HomecareHomebase, and collaborate with physicians to adjust care as conditions change.

Qualifications

CA RN License is required.
HomecareHomebase (Pointcare) experience preferred.
CPR certification required.
Driver license required in CA.

Responsibilities

Completes initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
Initiates the plan of care and makes necessary revisions as patient status and needs change.
Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
Assists the patient with activities of daily living and facilitates the patient’s efforts toward selfsufficiency and optional comfort care.
Acts as Case Manager and coordinates patient care for assigned caseload.
Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare).
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
Teaches the patient and family/caregiver selfcare techniques as appropriate.
Attends interdisciplinary group meetings.
Fulfills the obligation of requested and/or accepted case assignments.
