Job summary

Acacia Health is seeking a compassionate Registered Nurse to plan, organize and direct hospice services in patients' homes and communities, delivering patient-centered care with a family-focused approach.

The RN will perform comprehensive assessments, develop care plans, coordinate services for an assigned caseload, and document clinical notes in HomecareHomebase, while collaborating with physicians and interdisciplinary teams. California RN license required.

Qualifications

• CA RN License required.
• Compassionate and caring attitude.
• HomecareHomebase (Pointcare) experience.
• CPR certification.
• CA Driver License.

Responsibilities

• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Initiates the plan of care and revises as patient status changes.
• Develops a care plan with goals based on nursing diagnosis and family involvement.
• Assists patient with activities of daily living and supports self-sufficiency and comfort care.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Maintains and submits clinical notes in HomecareHomebase (Pointcare).
• Communicates with the physician regarding patient needs and orders.
• Teaches self-care techniques to patient and family/caregiver.
• Attends interdisciplinary group meetings.
• Fulfills the obligation of assigned case assignments.

Job Description Summary

The RN plans, organizes and directs hospice, and plans and directs services to meet the needs of patients and their families within their homes and communities.

Essential Job Functions/Responsibilities

• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Assists the patient with the activities of daily living and facilitates the patient’s efforts toward selfsufficiency and optional comfort care.
• Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
• Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare).
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Teaches the patient and family/caregiver selfcare techniques as appropriate.
• Attends interdisciplinary group meetings.
• Fulfills the obligation of requested and/or accepted case assignments.
