Mission Healthcare in North Orange County, CA, seeks a dedicated Registered Nurse to join our home health team. You will perform initial patient assessments, develop and revise care plans, and coordinate with families and physicians to deliver compassionate care.

The role is full-time, Monday to Friday, 8am-5pm, with a pay range of $58-$62 per hour depending on experience. Requirements include an active California RN license, 1 year acute care, BLS, and a valid driver's license.

Formación

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS), CPR certification.
• Valid driver's license and auto insurance.
• Bachelor’s degree in Nursing; 1 year home health experience preferred.

Responsabilidades

• Completes initial patient and family assessment to determine home care needs.
• Initiates and revises the plan of care as patient status changes.
• Regularly re-evaluates patient needs and nursing diagnoses.
• Develops care plans incorporating therapeutic, preventive and rehabilitative actions.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting plan of care and related needs.
• Provides health care instructions based on assessment and plan.
• Identifies discharge planning needs and implements prior to discharge.
